The 2i2 4th Gen's main line outputs now offer up to 120db dynamic range, compared to the 3rd generation's 108db. Likewise, the headphone amps also saw improvements. 4th gen can now output up to 115db versus the 3rd gen's 108db. All that to say the 4th generation Scarletts can output audio at higher volumes.

The output on the back of the focusrite is mono; I think the problem comes from the way it handles the two stereo channels and it's cancelling out some information between the two of them. On the pair of headphones I have with this problem, if I hold down the button on them that's meant to pause the music or whatever, then it sounds normal.
All you need is a cable that connects your chosen mic to the Scarlett 2i2 "combo" input jack. That's the XLR (3-pin) section of that jack NOT the 1/4-inch section. For almost all microphones than means a XLR(F) to XLR(M) cable. If you buy a phantom-powered condenser mic, be sure to remember to turn on the Scarlett 2i2 switch marked "48V".
